Change your payment or direct debit details

Want to change the way you pay? Use this form to give us your new bank account or credit card details.


Nominate a beneficiary – non super

Use this form to nominate or update who receives the benefits of your policy in the event you pass away.


Nominate a beneficiary –  super

If you have insurance through Zurich’s superannuation fund, use this form to nominate which of your dependents receive your benefits in the event you pass away.


Decline indexation increases

The inflation protection benefit protects the value of your insurance against the impact of inflation by increasing your insurance cover automatically each year. If you don't want this to happen, this form allows you to reject offers to increase your premiums.


Reinstate a lapsed policy

If your policy lapsed more than 30 days ago, but less than 12 months ago, and you want to reinstate your cover, you need to complete this form.

Insurance deduction notice for personal superannuation contributions

Complete this form if you intend to claim a tax deduction on the premiums you have paid throughout the financial year.


Non Lapsing death benefit nomination form

If you have cover through the Insurance Only division of the Macquarie Corporate Superannuation plan, completing this form will establish beneficiaries on your policy that will not lapse or change without a further form being completed to change the beneficiaries.


Notification of eligibility for superannuation contributions

If you are over 65 please complete this form to notify us that you are eligible to make contributions via insurance premiums on your policy.
