Business Expenses (as part of a Life Insurance policy)

Where the life insured is eligible for an income benefit and has business expenses cover, they may be entitled to up to 100% of the monthly eligible business expenses averaged over the previous 12 months.

How to lodge a claim

You will need to complete and return the following documents:

 Authority Form - Medicare
For the public to allow Medicare Australia to release personal Medicare claims information to a third party.

Ezicover

How to lodge a claim

1. First check your Policy schedule and Product Disclosure Statement (PDS) to confirm the Ezicover policy you have with Zurich. 

2. Contact the Claims team on 131 551 for a claim form. 

3. Complete and sign the claim form/s. 

4. Post, fax or scan/email your signed form/s.

Health Events

A lump sum is payable if the life insured meets the definition of one of a broad list of covered Health events, such as heart attack, stroke, cancer and many others.  Please contact your Financial Adviser or call our claims team on 131 551 to clarify if you have this cover.  To make a claim please contact us on 131 551 or complete the claim form.

Income Protection

How to lodge a claim

To lodge a claim, you will need to complete and return the following documents:

 Authority Form - Medicare
For the public to allow Medicare Australia to release personal Medicare claims information to a third party.

 Authority Form - Pharmaceutical Benefits Scheme
For the public to allow Medicare Australia to release personal PBS claims information to a third party.

Please also provide copies of any relevant medical reports or investigations the life insured may have.

For an indemnity policy, please provide full financial statements for the highest earning 12 months within the 24 months immediately prior to the date of disability.

Life Insurance (Death)

How to lodge a claim

We will endeavour to treat your claim with the sensitivity and efficiency it deserves. Please provide the following forms. If we require further information we will be in touch again soon.

  • Certified death certificate
  • Certified copy of will or probate (if there is no will then please provide certified copies of letters of administration)
  • Original policy document (if not able to be located please advise us and we can provide alternative documentation for completion)
  • Certified proof of age of the deceased
  • Details for new policy owners (this is required upon the death of the policy owner so that we can transfer the policy to the new owner who is nominated to us).

Terminal Illness

A number of Zurich’s life insurance policies include a Terminal Illness benefit in the event that you are diagnosed with a terminal illness and your life expectancy is less than 24 months. Please contact your financial adviser or call our claims team on 131 551 to clarify if you have this cover. To make a claim, please complete the following four claim forms:

 Authority Form - Pharmaceutical Benefits Scheme

Please ensure the life insured provides the original policy document and certified proof of age.

Total and Permanent Disability (TPD)

This cover provides a lump sum benefit if the life insured is totally and permanently disabled and unlikely to ever work again. If the life insured has been entirely disabled from working for at least the minimum period as stipulated in the policy (three or six consecutive months depending on the policy terms) and the life insured is unlikely to ever return to work, the following five forms will need to be completed and returned:

Please also provide the original policy document and certified proof of age of the life insured.

Trauma

Trauma cover provides a lump sum benefit where a policy-defined trauma event has been diagnosed or has occurred. Trauma claim forms and requirements are specific to the trauma event which has occurred. Please contact our claims team on 131 551 if you wish to claim under this cover.
