Total and Permanent Disability claims

Provides a benefit if the life insured is totally and permanently disabled and therefore are unlikely to ever work again.

Step 1

Determine that the life insured has been entirely disabled from working for at least the minimum period stipulated in the policy and that the life insured is unlikely to ever resume work.

Ensure the life insured has the following information available:
  • Original policy document
  • Certified proof of age of the life insured.

Step 2: Complete claim forms

Print out the claim forms and have the relevant party complete them.

Zurich Australia - Total and Permanent Disablement Form & Physician's Report

This form is to be completed by the life insured, only on the request of the Zurich claims area. You are required to have the Physician’s Report (pages 6-8) completed by your treating doctor.
